What happens first when you get a cut?
When you get a cut, scrape, or puncture, the wound will bleed. The blood will start to clot within a few minutes or less and stop the bleeding. The blood clots dry and form a scab, which protects the tissue underneath from germs.
How do you chart a wound?
Use the body as a clock when documenting the length, width, and depth of a wound using the linear method. In all instances of the linear (or clock) method, the head is at 12:00 and the feet are at 6:00. When measuring length, the ruler will be placed between the longest portion of the wound between 12:00 and 6:00.

Eschar is characterized by dark, crusty tissue at either the bottom or the top of a wound. The tissue closely resembles a piece of steel wool that has been placed over the wound. The wound may have a crusted or leathery appearance and will be tan, brown, or black.
Why is my wound wet and sticky?
Purulent Wound Drainage Purulent drainage is a sign of infection. It’s a white, yellow, or brown fluid and might be slightly thick in texture. It’s made up of white blood cells trying to fight the infection, plus the residue from any bacteria pushed out of the wound.
